HEALTHWISE: PHYSICAL ACTIVITY REFERRAL SCHEME (PARS)

PARS introduce individuals to the benefits of regular exercise and an active lifestyle to manage and improve existing medical conditions and prevent the development of related comorbidities. Our 12 week health programme aims to encourage people of all abilities to become and remain more physically active, replace unhealthy habits with healthier ones, and develop their confidence, knowledge, and skills to improve their health and wellbeing.

The Healthwise Team’s mission is to support individuals on their journey to a better quality of life by providing them with the most suitable exercise and nutritional recommendations.

All physical activity programmes are based upon participants’ preferences, medical condition(s), activity levels and fitness goals. Our Healthwise Instructors monitor progress and adjust physical activity programmes accordingly ensuring participants exercise safely and are on track to reach their goals. Healthwise is currently being delivered at:

  • Hammersmith Fitness and Squash Centre
  • Lillie Road Fitness Centre
  • Phoenix Fitness Centre and Janet Adegoke Swimming Pool

Who Is PARS For?

The programme is for Hammersmith and Fulham residents or patients from a Hammersmith and Fulham GP surgery. Healthwise is designed to assist people who are currently inactive with medical conditions or other specialist requirements that prevent them from entering mainstream exercise.

Common reasons for referral:

  • High blood pressure
  • High cholesterol
  • Diabetes
  • Circulatory / respiratory diseases
  • Fibromyalgia / Chronic fatigue syndrome
  • Depression and anxiety
  • Osteo / Rheumatoid arthritis
  • Back pain (post physio)

What activities are available?

PARS provides members with a wide range of group classes and exercise options to suit all abilities. A qualified GP Referral Instructor can help participants select suitable classes tailored to their medical condition(s) and needs. A physical activity programme may include some of the following:

  • Gym based sessions
  • Group exercise options
  • Water-based exercise

How can the Better Health Team support?

The Healthwise Team can:

  • Assess and monitor progress, providing suitable physical activity for       the individual’s condition(s)
  • Motivate and encourage participants during their activity programme to achieve their goals
  • Advise on how to exercise safely and with confidence
  • Provide assistance to exercise independently
  • Provide support and answer questions

What Membership Options are Available?

The Healthwise Membership provides access to the Gym, Pool and classes for a monthly or annual fee, obtainable for inactive individuals who have one or more medical condition(s).

You can choose from the following options: pay-as-you-go, monthly direct debit or upfront annual payments. Current prices are available on request.

 

How to apply

If you are interested in Healthwise PARS, please speak to your GP or Practice Nurse who will authorise your request. They will complete your referral form and email it direct to us. We will then contact you to discuss your referral.Please ensure your GP sends us the referral form direct.
